Physicians entering into the locums world for the first time tend to have many questions and reservations. Rightly so, making any major change in career and lifestyle would warrant some concerns. One of the highlights of my job is showing the major advantages of being independently contracted and being free to travel to different locations throughout the country.
The latest example of these advantages occurred with a new neurologist that applied to our company. She was looking for an opportunity to work half-time but still afford the same lifestyle. I told her this was definitely possible and the assignment that would allow the lifestyle she wanted would be in Nebraska. Never visiting the state, or even the region, she was very reluctant. I explained that a huge benefit to locums was that she could try the facility for a short period of time. If she was unhappy for any reason, we could simply book her somewhere else the next time.
Upon submitting her and conducting a phone interview, we realized her mentor from residency that she lost touch with was the administrator at the facility! Six months later, she still loves traveling across the country for 2 weeks out of the month. She spends half the month with esteemed colleagues and has half the month to devote to family.

Hospital medicine tops the list of assignments again this quarter, with a 63% increase versus one year ago. Few primary care physicians make rounds today, so they’re turning inpatient care over to hospitalists. In addition, 28,000 or nearly a quarter of all internists have left primary care to become hospitalists due to Medicare/Medicaid cuts. Approximately 60% of all U.S. hospitals and all managed care organizations directly employ or use hospitalists through a locum tenens agency. Both the physician and facility benefit financially from a hospitalist program. In the last year, providers experienced a 6% increase in hourly rates. In addition, hospitalists working 26 weeks a year on a seven on/seven off schedule will generate $25,000-$35,000 more than in the same dynamic for a permanent position. The trend will continue in 2010, as estimated demand will call for an additional 10,000 hospitalists nationwide.

The internal revenue service has announced the new mileage reimbursement rate for 2010. This year the mileage rate will be dropping from fifty five cents per mile in 2009, to fifty cents per mile for the 2010 year. The change in rate as stated by the IRS is due in part by lower transportation costs as compared to a year ago.

From my three years of experience in the world of physician staffing I have found the true value in looking for the “silver lining.” This poetic sentiment alludes that even the gloomiest outlook contains some hopeful or consoling aspect. The everyday routine for anyone involved in medical staffing can be found to be stressful, fast paced and mundane at times. I wanted to take the opportunity to share what fires my drive as a physician recruiter and allows me to find the reward required to not burn out.
This year I have had four of the locum tenens physicians that I represent find permanent placement through their locum’s assignments. As a locum tenens recruiter you often represent your same physicians for an extended period of time and develop a good understanding of and friendship with the providers you consistently work with. It is these connections that develop along the way that add a personal touch for both parties and a real sense of reward for the recruiter whenever they are able to make a difference in the life of our providers. One example of this that I would like to share dates back to a year and a half ago.
I first made contact with this particular physician when she had just completed her family medicine residency. She was torn in attending a hospitalist fellowship due to the location and downtrodden community. I was luckily from this region of the state and got her set up in a neighboring resort community that I had previously lived while in grad school. We maintained contact throughout the following year although she had never sent in her credentialing packet to sign up to work locums.
I was persistent and finally got her to fax in the paperwork required a month before she completed her fellowship. At this point she had informed me that she had a permanent opportunity lined up in Oregon but was not too excited to leave Texas. She discussed her difficulty in local interviews since she was a family medicine trained hospitalist and Oregon was her only option at the time. I was pleased to finally have to opportunity to get to work with her and started searching for some in state hospitalist locums while her Oregon license was being processed. I was able to get her accepted at a facility with an established hospitalist group for several shifts a month at a competitive hourly rate.
After working at the facility for her first round of shifts, I received an interesting phone call from her revealing they have offered her a permanent position. This contract allowed for much higher annual income, better benefits and the opportunity to remain close to family and friends here in Texas. The excitement in her voice and overall positive change in outlook said everything. The gratitude she expressed and the knowledge that I was able to make a difference and positively impact a provider’s life is everything I need to walk in the office door every morning.
With any situation in life it is often easy to fall in slums and let the stress of life become overwhelming. Focusing on the “silver lining” and positive aspects will always carry you through. I am a locum tenens physician recruiter and will jump at any opportunity to help a provider because you never know where that journey will end. The potential benefits for providers far exceed any excuses for not trying.

Recently one of our Senior Recruiters, Dave Brandner, told me a story about one of his docs that I have to share. He was at a pub in Dallas and saw an old travel chest with the name Hipskind on the top of it, the same name of one of his locums physicians. He had a meeting in Dallas about a month ago, and he and Dave made plans to go out and have a few cold ones. So, he picked him up at his hotel in Grapevine and drove him down to the Londoner in Uptown to show him the chest. He was pretty fascinated, and said he’d love to have the chest. On the ride home Dr. Hipskind was still talking about it.
The next day Dave called the Londoner, got the name of the owner, and asked him if he’d part ways with the chest. He said sure, but that Dave would need to pay for a replacement furniture piece. So, they came to agreement on price, picked it up the next day, packaged it, threw a 6-pack of Coors Lite in the chest and sent it off to Dr. Hipskind’s home in Illinois. He was very grateful, and insisted on repaying Dave the expenses associated with acquisition and shipping. He did accept the 6-pack as a gift, however.
Dave, thank you for going above and beyond to provide Dr. Hipskind with a memorable customer experience!

Dear Mrs. Sherri Bowles,
My wife and I felt very welcome and we acknowledge with much gratitude the warm comapny of all the people of Delta that were present at this event. The warm reception accorded us by everybody was so overwhelming, my wife was asking for us to move to Dallas!! See.. I was going to say "it doesn't take much" to make us move, but then it was "much'. We were surprised by all the amenities you provided us. It was a big surprise to us ..the front desk surprise at checkout at the Gaylord was not the beginning but the icing on the cake. You even went so far as to provide us with a great foursome at the golf outing..ha! We had fun. Jason, Mark, and David made a nice "walk on the park" on a hot Dallas afternoon so much fun and memorable....and for me especially , the heckling ladies on the front 9 par 3 hole were fantastic (shhh!!).
We knew we were very welcome even before we made the flight. Jason is a kind of person who has a knack for making you feel comfortable. I had that feeling about him from my professional relationship with him. That was the reason I signed up with Delta at that time. I had a recruiter from another company talking to me about that same time. I was going to join a fee for service practice in Ohio and I needed locum jobs for income while I waited for accounts receivable to materialize from my new practice. One day I got a call from Jason Ard about a reference for another physician. I had not yet filled any paperwork from the company I was talking to for locum positions. Jason spoke with me and my wife that same day he called me for the first time. My wife was impressed by his professionalism and so was I. He came off so calm and professional, I recruited him as my recruiter! True story!!
This has been a one of a kind experience for me in terms of professional relationships and I believe if you develop a model such as this (getting providers involved socially as well in campany events) with little tweaks here and there you would be peerless in this competitive space. You have the resources...the staff is great, professional and friendly; top to bottom. You create a sense of belonging and that is what you need from providers, not just as tools for extra cash here and there. That sense of belonging is what Miriam and I took away from Dallas; it was solidified. There is no question we are coming back to play next year. David already booked us.
We appreciate your thoughtfulness. Please extend our deepest appreciation to the staff. This was what I was going to say at the awards ceremony if I had time to say anything..had we stayed for it. So there you have it. Thanks again and see you next year.

Consider it as a working interview if you are looking for a permanent
change. It allows you the opportunity to actually see what it’s like to
work in that practice/facility. See how well you adapt to their ways of
doing things. See how well the staff gets along and interacts. Find
out if you fit in with their culture/environment.
Can you imagine
yourself working there long term? Spend time touring the community.
Is it a good fit for you and your family? Does the community provide
activities that meet your interests?
Consider locums opportunities not
just as an opportunity to supplement your income, but as an opportunity to find
the best practice that suits your needs on a long term basis.

There's an interesting article on the MSNBC page this week that actually contradicts articles that I’ve read and even
written about lately. While most parties agree that ER visits are up
across this country, some may argue that hospital admissions are up due to the
high fees associated with treatment. The article discusses that
individuals are refusing additional service in the hospital, going against
medical advice, based on the country’s current economic situation and the lack
of funds people are willing to spend.
